Provider Agreement

This agreement covers healthcare providers, brokers, and managed care organizations arranging transportation through Ebosi.

1. Services

Ebosi provides non-emergency medical transportation, ride-hailing, and courier services as described in your service order or master agreement.

2. HIPAA & data

Ebosi enters into a Business Associate Agreement with each provider and handles protected health information only as needed to perform the service.

3. Billing

Trips are billed per your negotiated rate card and the service level delivered. Invoices are issued monthly unless specified otherwise.

4. Compliance & reporting

Ebosi provides trip logs, signatures, on-time reporting, and exception reports in formats acceptable to your payer.

5. Termination

Either party may terminate with the notice specified in your master agreement.
